10.10.10-I got to make an entry today!!what's the big buzz about this day?I've no clue but all i know this so called 'i-dont-know-why' it's so important date is just all over my fb feeds..so let's make it a 'memorable' one by publishing an entry.

Recently,I'm having Forensic sciences cycle. It's been almost 2 weeks and I only have another 3 more days to end this cycle.Pretty much sad, because I'm actually enjoying this cycle very much..ask me why?for a couple of reasons I would state-1stly,it's my most relaxing cycle(no preparation is needed before classes the following days, unlike many other subjects),2ndly-forensic science was my option besides medicine during my form 6 days and 3rdly-the classroom and what we do in the class is way to cool--:), 4th-this branch of science plays a major role in medicine= in a criminology aspect:) 5th-the lecturer(we got his name all wrong in our case reports)..darn,and yeah he was actually a very easy going guy, compared to how he is in lectures..very schematic indeed and he actually checks the attendance one by one.One setback: it's very hard to rewrite whatever he was reading out(he reads only once) and his terms and pronounciations~

1st day of class-AWESOME,it was my first day of live autopsy.Corpse was found dead in his apartment.I got a better insight on how they do the post mortem from top to toe, from external investigation to inner investigation.56 year old, alcoholic man, with vomitting mass on his shirt, mouth, hemorrhages in his eyes, edema of the brain and lung.the final diagnosis was:death due to aspiration caused by alcohol. We passed up a case report after a week. And I realised, there's a very thin line or maybe there's no difference between a butcher and a forensic doctor.

the following practical classes were only in the class.THe class room is creepily cool with dead models on display, type of weapons used(real ones),schemes of how to make diagnosis, type of wounds found on the corpses, extracted organs with defects~

As much as I'm enjoying the classes, soon I will be having a very hectic cycles coming up-next week on thursday onwards will be started with occupational diseases for another 6 days before getting myself fully committed to internal medicine.

I think I missed out this prime line---"OMG,IT's ALREADY OCTOBER!!!!":))))))))) *big and wide smile of mine*

over the years...

-I must grasp more-
1520 @ 31st July'10

Screw me for posting an entry almost a month ago and finally after a request from shalu,I decided to make an entry today.A month has gone and another month is on its way..august!!!Not looking forward, wasn't expecting time to pass by me fast, but on another thought, this time,it will be my last and final 'go-back' to my 2nd home: Russian Fed for my final year of my 6 years of medical studies.Happy, extremely happy but I know for definite that I will be missing Nizhny, memories of being abroad +medical studies+new friends+ETC+euro tours are the very few things which I will be missing much.I've had a fair share in all these.

Another phase of life is waiting for me eagerly.A Doctor's life.I know it won't be any easier.I used to mourn during high school days,"damn,I'm stressed..."...during form 6 :"man, how am I going to get through these subjects? It's way stressful than form 5"...But now in Medic line,I realised the true meaning of "STRESS".That's when I came to my self-realisation" "should have studied more harder, yes harder and harder..." and now the dissatisfaction still lingers in my mind whenever it gets a chance to invade me..medic studies,no joke -i always have these 'threatening voices' in my mind saying -you gotta do well, you will be dealing with lifes----what could be more frightening than this right? The mass evolution of medicine nearly every day proves the fact that a physician has something new to learn every day.Being a Junior Doctor,I won't claim myself as the superior one.Still,I would admit humbly:I still have a long way to go.6 years of studies which will be coming to an end next year in June,is just the beginning. The real drama of my life will enfold soon...

JUly'11,I know I will be busy preparing all the necessities to start my new life.I know I will be waiting more anxiously to receive my posting letter from MMA,I know I will be packing to move out (if I ever get posted to elsewhere besides in ipoh),I know I will be spending most of my life in hospitals,I know I will be having a very little time for my family and friends,I know I wont be able to just drive and meet up with my buddies at a mall after a ring(like now!)...a busy bee's life(perhaps 100 fold than that is peeping at me)..I chose this, yes I know but now for sure I can tell that---for the past 22 years of my life;books have played the major role and life has become more serious,I wish it's less serious and less demanding(slight regret is there,to be honest ).

Medical studies and memories cannot be separated.Memories can keep us alive though at times it can be pretty ugly. What all of us need is a memorable one.So the best is,keep the beautiful ones which gave you the happiest moment in life and throw the rest away.Things and people will change, hence don't compromise--you got ur life to live on,let the other person to mend and change their life for better.Nature of forgiving shouldn't be taken for granted...stand firm with ur decisions, don't hold any grudges on others(even on ur enemies),focus in what u have to do and be who you are,but when you have to EXPLODE,please don't bottle up ur anger.Let it go.

I'm not a philosopher wanna-be,just an ordinary girl..I'm not superior/great but I will always remain as me for years to come.I don't throw people out of my life but expect the unexpected when you mistreat me.:)

Well,I'm very much looking forward for tomorrow for an outing with aud,sathia,vicks..5 days at home with the idiot box playing and airing some crappy movies,is just too much to bear with..can't wait to see the rest tomorrow..gonna have a long day outing and surely will be missing those absentees;shalu,sree,subha,anne....okay,signing off for now.

At the Extreme Hiatus...

*do we look professional enough?*-days of ophthalmology rotation (photo by-sarah repin, thank you:) )

This blog post of mine might be the first and the last for the month of May.I need plenty of extra hours each day(my ever humble request),when it will be granted?I know I'm back in here after an interval of 1 month..neither I am left without anything to be blogged nor losing the passion for words, but the most explainable reason is-I'm at the RUSH HOUR of the year. The time has come, with just 2 weeks away before exams of the 10th semester to commence. With not any lighter or lightest subjects,I've been balancing these two papers.Least had I expected that Infectious Diseases exam will wall just a week apart from the massive paper of the semester-Internal Medicine.

Ophthalmology rotation officially ended a week ago when I completed the exam last monday.I enjoyed learning about the eye under the associate professor. The days of ophthalmology were fun when I finally got my hands on the slit lamp, ophthalmoscope, learned how to measure the visual acuity, visual field.Yet,any invasive procedures(ophthalmic surgeries),parabulbar and subconjunctival injections will surely to send chills down my spine. They never turn me off but just the thought of 'how could these delicate organs stand such traumatic(?) procedures?' Dear Heart,I still root for you no matter what!!!:)Nevertheless, for the purpose of learning,I'm in for ANYTHING.

Madam Dugina's sacred line " Discipline is the mother of success".Guess mine has doubled or increased more once the month of May begun.Others can wait but I'm not denying, I'm missing something and I know what it is:).

You got to excuse me now,I will be having only 2 hours of sleep today before I get ready for my 1st day of Gynaecology rotation. It's going to be a long day with Internal lecture till 4.30pm.And I'm still awake?--gonna hit my bed now.At times even I myself, very much amazed at the amount of energy I have in me!!!

Good night(good morning).

*I'm thirsty for knowledge and I want to learn more*

I,The Sombong Girl(apparently).

" being the most sombong girl?-anyways,it's fun"-Navanita :)
26th March'10 @ 1400

It's been real a tiring week, even when I had only 4 days of classes since Monday was an off day due to cycle break(an off day before I started with my Internal Medicine subject!)Perhaps it wouldn't have been this exhausting if it wasn't for our super strict and hidden sarcasm doctor. She definitely has something against me.Not a day goes by without her picking on me for even a slightest reason..sigh...2 more weeks with her, gotta get through it.Nevertheless, having her as our practical classes' mentor is one thing my group has been looking forward.Well can;t blame her for acting that way when her previous mentor was the most digeruni Dr. in NNSMA!

Emergency cycle with anaesthesiologist,Dr.Pitchugin in the evenings. How does it feel like having to squeeze your brain/on the go around the clock from 9am till 4pm without lunch to feed your cells? That's what exactly happened yesterday..by the time I got back home,I was too drained out.I know I had to eat for I have been 'fasting' since morning but the feeling of hunger was supressed further.Had a bun, checked my mail and there I went, under the blanket till 8.30pm.After refreshing,a moment in fb with replying msg, keeping myself updated with the latest on TheStarOnline,at 11.30pm,I faced my books..topic for today:Differential diagnosis on abdominal pain, prescription list for my patient of the day and doses on hypertensive crisis drugs,I had to call the day off at 3.30am as my eyes were refusing to stay awake though I wanted to stay vigile till the morning.

For 2 days in a row,I had been getting up unconciously, turning off my alarm and dozed off.Luckily,I still got up at my usual 5.30am and hit the shower. The accumulated tiredness is taking a great toll on me.No,I'm not grumpy but out of I feel everything has been hitting back at me directly right on my face!!!Never ending thick books to flip through...hang on,Nita!!!

Besides all that the week has been good.Had a good chat with Ragu on sunday till 4am Malysian time, received 2 good messages via fb.One of it I have to mention it here,..Prakash messaged and informed me that his engagement will be in July once I'm back. His pre-engagement day will be officiated with family reunion and bbq which will be held in Teluk Intan*loving it!*..I don't think any other place will be ideal for him to intro his would be to The Family.I'm happy for you, cousin bro!!!

We will rock it this july!

I'm not a girl who wears a smile all the time.Though my threshold of turning into a red chilli face has increased(miraculously!),yet the onset is still considered very much alarming.I have no autonomic power on my emotions,so just bear with me,with an addition of a new title: the sombong girl.Thank you.

Okay, gotta leave to hospital in 30 minutes time and T.G.I.F (Thank God It's Friday!)

a toss for.....

*SMILING?EMOTIONLESS?SAD?@LABILE MOOD?*

This is my post 1 week of exam blog. With the commencement of 10th semester,in a week I had psychiatry exam.Exactly one week right after my exam.Yeah,I got through excellently, more like my mark was given based on my performances in class and with an additional question(I've never been asked that question in any of my exams here!).Done with my 1st exam for the sem and currently into Infectious Diseases rotation which will be ending this saturday.

I was waiting anxiously for Friday(19th February'10) so that I could take a complete break for the next 4 days(which I did,3 days without books!).23rd February is a public holiday which marks for Men's Day.In real, looking back at history, formely this celebration was intended for those who had served the army and it was known as Red Army Day.Basically,23rd February is celebrated by giving gifts to all the adam's species in Russian Federation,Ukraine,Belarus.Officially now,it is called DEFENDER OF THE FATHERLAND DAY.And now, tomorrow, back to my usual routine..ermm,can I extend few more days of break?Going to bed when the dawn breaks, and waking up past noon,is certainly blissful!!!:p

Chinese New Year celebration was held yesterday at 3rd hostel. No performances like the previous years, everyone is certainly BUSY!There's no fun in just thanking and wishing "happy chinese new year' and walk away with food/just stay inside a room with a couple of friends.I kind of missed our 4th floor's spirit..I still remember 3 years ago in our 5th sem when the whole 4th floor's residence(atleast majority of us) sat on a mat spread on the floor and enjoyed the food, that's how it's supposed to be but sadly,it's not like that anymore, especially now with another 2 semesters left before we bid goodbye to each other.

Anyhows, the food was really tasty.Rumin sim gave me this large piece of fried chicken...*burp* thank you everyone:)

On sunday, for the very first time,i tasted yee sang which was prepared by yeow, kok meng, soo ching, rumin, kheng huat and shari.Tossing yee sang was thrilling but not when it falls on others hands!!Thank you, thank you..too bad the pix are still with Kok meng, can't upload them:(

So they say it..Happy Valentine's Day!

*on this Valentine, for my valentine,I give You my heart*-I think this is a very cute pic :p

"Nava, what's the plan for sunday?" Jeff questioned me to which I responded with a smile.Still the both funny groupmates of mine, who are apparently my walking partners from psychiatry hospital(every day!) continued asking "You know what's the day right?" This time I responded back saying "No,I don't" obviously with a smile, again. They didn't believe though saying "ah,you don;t know?" and burst out laughing! Jeff again "what you will be doing on sunday?" Me: "will be dating my psychiatry notes. That's the plan for Tuesday's exam!"...so here you go,the time of the year came again for us to show and spread love. Thanks to St.Valentine for making it an official day to celebrate the day of love, but, just the me asking again a very simple question:"I thought everyday is a Valentine's Day,no?"

10th Semester had been officiated with psychiatry exam cycle..yeap it has been a week, and next Tuesday will be my first exam for the semester. So still trying to get all the psychiatry terms into my mind. The first few days I found it very hard to get started with books after the extreme hibernation and idle me of 10 days of holidays.I was complaining "WHY ONLY 10 DAYS?".."I WANT A LONGER WINTER BREAK!!!".But now,I find psychiatry is interesting, too though not that much when it comes to patients, well will come to that point very soon...

Previous semester,Dr.Kitaieva was our mentor and for this semester,Dr.Ivan Borisovich took over and he will be our examiner. He's very good at commenting and explaining and the best thing about him is he himself being the 'patient' especially when he mimics and demonstrates gestures, physical appearance and every day situations of a psychic patients.Good job,Dr.

Last semester, the whole cycle was spent in our classroom wherelse this semester,we were exposed more to patients.2 days consequently (yesterday and today) we visited the female department. All of us had the fear (atleast the slightest) before we were even told to change our second pair of shoes and get ready to face the patients.

Yesterday was my very first time being in the department.I was on my guard,so did my groupmates. As the main door was unlocked to let us in,the very first female patient we say exerted the fear in all of till the end.She was in her green robe, with a continuous serious look, with subtly flexed face, looking very intensely at us.We walked pass the many patients wandering outside their wards and a few even came out from their rooms upon seeing the 'new doctors'.I was standing beside the couch seated by sarah, xiang yun and treasure, when we saw the lady in green was advancing her pace towards us with a manner to attack. My heart started to pound and when another patient(whom later was interviewed) approached us,she turned away and was back to her initial position standing behind the locked door.Phewww!!!

3 patients were interviewed.Everyone with 3rd auditory hallucinations.I wonder what the voices been telling them while they(the patients) were facing us.The very much anticipated abdalla became as quiet as a mouse upon seeing the patients.Though with delusions, agitation, tangentiality, flight of ideas which can annoy others, they still need the fullest care and support to get better because there's no way they can manage it/recover with only medications,or most of the time there;s no history of full recovery in psychiatry patients.Today,we had 2 patients with Alzheimer's Dementia and Vascular Dementia.2 different patients(totally).Impairement memory and intelligence are clearly visible when the clock could be drawn(to access visiospatial) but failure to point it at 3.40pm.Both patients couldn't even remember their children and spouses names. How saddening is that now right?Alzheimer;an unknown ethiology of progressive impairement of the brain's function can give a different outlook towards life to appreciate and be thankful for being the gifted ones once you're exposed to these type of situations.Still,no one can predict who will be the next victim of Alzheimer's as the age catches up.

On another note,on monday and tuesday, zakiah and I were followed by a young lad in his early 20s by the name Dima. From my observation,he is definitely a patient over there. His movements were rigid(like a robort),bright red eyes but well groomed and with a slow speech.Initially, abdalla became his first friend after I passed by him and went up upstairs. He followed abdalla and wanted to get to know the girls.Luckily, all of us were saved by Dr.Ivan Borisovich ushering us into the classroom. The next day,as I entered the department,he was waiting over there. He came so close that I started to get anxious. As the approach towards psychiatry patients are more 'gentle',I remained on my position and came a question "what's your name?" "Leena",I answered to which he replied "nice name"."On friday, can you come for a concert?" Me: "I can't,I have classes".Dima:"Okay, after class, you come". Me:"okay".I was just waiting to get out of his sight and quickly climbed up to my class.Later during break and again as zakiah and I were at the washroom,he appeared again asking zakiah "Is Leena in there?"..Came out,he asked me something which I couldn;t undertsand and I said "bye" and left. Now,isn't that a freaking good experience to have had an encounter with these patients?

To all those are reading this entry of mine: "Happy Valentine's Day to all of you".And remember,Valentine's Day is a day to show your gratitude, appreciation and love to the ones you kept very very close to your heart and carry them wherever you go:) Celebrate the day and if possible, make it an everyday event, and not only for a day(14th February each year!)

9th rotation:Infectious Diseases.

I've never been this exhausted ever in my 5 years here, but yesterday was an exception. The week ended with a gush of tiredness, which made me so worn out at the end of the day. What a Friday was yesterday!!!

All the tiredness of the week exerted 'the cumulative effect' yesterday.Firstly, blame it on the 6 days of quarantine which threw my plans off the track.BECAUSE OF THE QUARANTINE,we have to replace classes of epidemiology with the consent of the professor.Actually I don;t mind replacing all the classes, and THAT'S ONLY IF she's in charge. Her extra notes besides the usual read outs of lectures by other teachers, makes her lectures interesting for me.

As for that,Group 531E,F and D were only asked to replace 2 days of lectures. With new rotation on infectious diseases from 9 ok,latest 9.30am to 2pm,we had to leave to minina for epidemiology at 3.30pm till 5.30pm.On wednesday, the class ended at 5pm.By the time I reached to hostel,it was already 7pm...that's my friday!!! I came back, cooked my staple food, maggie + egg,had my so called dinner whilst surfing the web and by 8pm i was all set to zzzzz...

and today,I woke up at 9am! A complete 13 hours of sleep.Did some room cleaning, had breakfast and I think I will be going back to my bed very soon..,perhaps after a couple of hours because the tiredness in me is not weaning off any time..duh!that's not a good sign when exams are just around the corner..errmmm...

Infectious diseases rotation.Professor is really good till I had to scribble her lectures and rewrite them back home if I were to copy each and every word of hers. So,thanks to my E66,whenever she goes out,one of my groupmates will be guarding the door, and I will carry out my mission to snap her lecture materials for future reference...

Personally i think, they should have just combined epidemiology + infectious diseases cycles together. Why?Mostly because the topics of infectious diseases are related!(in fact the exam questions for both of the subjects are the same!)...so,they should have just compressed both cycles at a time, but Russians...what's more there to be said?

In class, practically we just discuss whatever that's been given in lectures and maybe add one or two new informations from our tutorial classes' teacher..pretty much less hectic cycle for me.So now I have an ample of time for the other subjects which I had given break for few days due to my busy days...

Okay,back to week,eh..maybe after a few hours of nap?will think about it...~

8th Rotation: CARDIO SURGERY.

4th December'09 @ 2234

At last, here I am,sitting on my bed, leaning against the wall,me DELL on my lap, playlist with my favourite tracks and FINALLY this is my first post for the month december which will be officiated by my most waited and looked forward rotation: Cardio Surgery. To be honest,in my time table it was written surgical diseases and I cared less actually, thinking my group, just like any other years will be travelling all the way to Ilinskaya and for the 3rd time(maybe?) will be stuck at the Urology department but hey,I gasped and said "REALLY?WE HAVE CARDIO rotation?cool...yeay!!! "

This cardio rotation is from 2nd dec till 8th dec *sighing here*..why so short of time for a most vital organ,am I right? Can we retort and go on demonstration here so that they will abolish subjects like philosophy and hygiene and make use of those times with Cardiology?*how I wish*...You can excuse me here for being extra sensitive when it comes to the heart,I can't be helped here, addicted completely with these complex organ for years and I know for sure which speciality I will be choosing once I'm done with my M.D.I've never changed my mind and that's what I want. The strong affection of mine towards the heart can never be separable...I'm just totally in love with the heart as an organ!:)

1st day of rotation:prepared for class on anatomy and physiology of the heart as we were told by the previous groups. We will be under a nepalese doctor.Zakiah was all agitated wherelse I was more relaxed and cool that I even took a pic of myself at my study table(he-he).Zakiah: "takutla, cikgu nepal tu sarcastic tau"..Me :chill la,it's the heart..I'm al excited..see look at me!!"..Zakiah : "yela, you suka the heart".. me :"yeappppp" and continued " after having all the strict teachers, tak kisah dah, let's see what happens today!"

Reached cardiac centre on time but as we entered Dr.Krishna's(if I'm correct)cabin, wani, syahrul, fil and sara were already in their positions.Names were already written on a piece of paper and began ice breaking ceremony..then first thing he said "come before 9 am!"..At one glance, looking at his fair skin, petite and thin physique, very neatly combed hair, and spectacle on his nose, you'll think he's just like any other doctors(go easy on you) but I know, you cannot fool around with him. I just knew it even before I saw him..why?Because you're talking about someone in the field of CARDIOLOGY!They just have this 'arrogance that attracts others and it will actually inspire you to move towards your goal!'+ with this unique aura of being a CARDIOLOGIST/CARDIOSURGEON.If it was any other doctors with full of arrogance,by now I would have critised them..I know I'm being so unfair but CARDIOLOGY IS THE SPECIAL FIELD AMONG THE REST.

Towards the end of the class of our first day,I was brain dead which happened the following day and even today..yeay finally it's friday! This is the most stressful rotation that we have ever had in our 5 years here especially when it comes to surgey cycle. We are only given 30 minutes of break and class will end sharp at 1.30pm.One by one will be called to answer his questions without referring to any notes and he will catch you and hit you back directly on the face when you failed to answer him, just like,he'll go "see, you didn't read the books,if you had read, you would have known the answer!" and that made my blood pressure spike up because I simply anti anyone who says that to ME!and yeah it changed our opinions on him.2nd part before the break,we visited the patients,I was the first to take a full history complemented with physical examination on a patient with congenital bicuspid aortic valve. It was ok but as usual, the language is the barrier..and as for the practical part,I did well (yes!) ..later,we moved on another patient..and our final task during the 30 minutes break was to make a short case report for discussion before we will be dismissed. In addition to that,we had to make an article containing >8 pages of information related to our given topics. Now you see..this dr is hyperactive and strict despite his stature just like they say "kecil-kecil cili padi" or "diam-diam ubi berisi"

2nd day : the nepalese dr. who's doing his second year residency here, continued with his skills to drain all our energy with his questionnaires. He's very good in trapping you...he'll listen carefully without any facial expressions or gestures, then to recheck whether you are being attentive,he'll ask you to repeat what your colleagues had just said. When you fail to repeat,he'll give you his wisdowm words " always respect to listen"..He lets you learn your mistake without pointing at it for example,he'll listen to " pumonary veins drain to right atrium ,right?" and you'll go "yes,it is" *in the midst of answering his questions/during presentation* and then he'll just smile and ask another person to alert the 1st one..see?He showed us echocardiography of valvular heart diseases and pcitures ofpulmonary amboli that was removed. He even took out from the refrigerator and showed us all the 'pathological artifacts' which they had removed during the heart surgeries. same routine continued yesterday :presentation-patients-ad hoc case reports-discussions.The number of scowled faces increased today.

3rd day : My presentation today based on atrioventricular canals. It was 10 pages long.I prepared well (I think) that I didn't had much time left for my today's topic : Ischemic heard disease.IHD is not something that will leave my mind easily after the strenuous cycle and exam of internal medicine,so I was confident that I could answer him if any questions were to be directed to me.After patients, break and dicussions. We watched clips on coronary angiography, how is it done, and it's indications. He was less uptight today, was making jokes and made us laugh when he still bombarded us.Words of wisdom for the day" where there;s a will, there's a way" .."learning is a continuous process".."listen to your patients"..Finallt when we were done for the day ,he sat back,on his chair and asked us to give comments on his classes, both positive and negative ones. One thing all of us had in common: WE REALLY GAINED ALOT IN THESE 3 DAYS.(like what jeff said :it's not even the 3rd day,I'm already so exhausted!)..so when it comes to my turn,I said:

positive remark: This is the best surgery cycle ever.

suggestion: can we atleast watch 1 open heart surgery?(to which he said we might be*fingers crossed, hopefully!* on mon /tues)

negative remark: *looked at one by one * and I said .." we don't like when you say WE DIDN'T READ THE BOOKS!" (to which he smiled)

His response to all of our comments :ACCEPTED!

conclusion: He's the best dr.anyone could ever get here in russia. If only all the teachers are like him we will be great medical students and inevitably a competent one.*Believe it,we said that to him* Fil even said : Wish we could have met you earlier.Just like I had said, his arrogance didn;t make us feel alienated/angry towards him..it's just that your ego is challenged when he smirks at you. He's very sharp though.Definitely,he was the all studious type during his med school years. His knowledge on CARDIO is adorable and it's the way he explains them without any hesitations when you're in doubt.Maybe his fluency in english despite his ascent makes me write all this admirable characters his in CARDIO.

As for me,I wasn't under a great amount of stress just like how the rest had mentioned. All that I see is me myself being like him one day.I want to be some one like that. This doctor is another inspiration of mine which has started ever since wednesday.

We can't thank you enough you,sir though you made most of our hearts palpitate in your classes.I'm very proud to say,I'm his last victim which he had picked on(the least number of times,I would say)..and I;m very happy for that.

Just like Dr.Krishna had said "BEING A DOCTOR IS NOT AN EASY STEP"..and now,who will deny that?I know,I still have a long way to go in this path of CARDIOLOGY.

Once again,Thank you DR.

6th rotation:Orthopedics & Trauma

12th November @ 0735

Arigato Gozaimas(good morning in Japanese:)),bloggie. This is my very first blog post early in the morning just before I leave to the trauma hospital:)

To nascent with,I'm a bit overzealous here because tomorrow I'll be having a day off..actually only the girls will be having break after a short and 'ad hoc' notice was out yesterday from the deans. The boys(all of them!) have to get vaccinated against rubella infections due to the increasing number of male students being affected by the rubrovirus. It all started 2 months ago when one of our coursemates were diagnosed with rubella infections. But lately,it just seems the boys are having rashes on their bodies, fever. Therefore, boys get ready to be vaccinated tomorrow and stop infecting the others,okay?lol..and yeah, thanks to all of you for the 'unofficial' off day:)

Orthopedics and Trauma- today will be my 3rd day of tutorial.1st day, since there wasn't any classrooms left to inhabit us,we only had a very short ice-breaking session with the traumatologist and by 10.30am we were free.So officially,2nd day we started our lesson with watching a recorded service done by the Red Cross Association in Sudan, mainly contibuted by the Australian,Sweden,Kenyan Medical Professionals.

* to be continued*

*continuation*

here's the next part of my post which I was supposed to post it earlier in the morning!Back to square one maybe?maybe not, let's get back to the movie that I had watched on war surgery. As i was watching the medical clip,I was thanking God for how lucky I am/how fortunate my life is.Looking at the victims, waiting for hours under the imminent sunlight for the arrival of the helicopter to fly them to any nearby hospitals.I remembered how we the lucky ones will turn so impatient and grumpy in clinics/hospitals when we are attended 5 minutes late. But these victims were just greatful for the medical aid being provided to them that they actually co-operate with the surgeons and physicians.Perhaps they had undergone too many traumatic incidents in their lifes that even a little kid didn't utter/let go of a shrill cry when treated. In short, the victims of the 3rd world country make the most out of whatever they could get. After all,the war has to go on in order for them to exist right?

After the movie,we had a brief discussion on triage and medical management regarding war surgery and later we were dismissed with hometasks on bleeding and antishock treatment.

today(12th november):started again our class with a recorded movie on amputations,graftings,fasciotomy,..and we got the idea that each class of ours will be officiated by a movie before discussions on hometasks. It wasn't exactly a movie which you will anticipate at 9.30am.Who would, please raise your hands:

description of the movie:

location: Operation room(OR)

objectives: amputations

fasciotomy

skin grafting/plasty

additional comments: eerie feeling (just like when you watch the movie SAW)

totally gross(looking at the wounds after explosions)

man= lifeless object(in skin grafting: it just looks like using a peeler/a grater for carrots, using of mini chain like saw in breaking the bones, screws, spanars to tighten the screws...)

I'm not a phobic when it comes to seeing blood/a pool of blood.I can always get away with it as long as the patient is unconcious.Whilst watching the movie, many couldn't watch the surgeries performed in silence:one can actually feel the sensation of being grafted on their own thigh for a skin plasty, the oozing blood (fluid like hematoma) can regurgitate your food upward ,later outward. So the best way to watch surgeries is to be in empty stomach, treat the patient as an innate object,be emotionless with the only motive:PRIMUM NON NOCERE (first,do no harm)@ SAVE LIFES FIRST!

Surgery is something I've always fascinated.I'll just get drawn to them what more if it conceerns about the cardio.I have a dream to watch a open heart surgery (live!) in Russia before I graduate, hopefully my dream will come true very soon;) oh did I tell you that in my 3rd year,my right hand was just above THE HEART of a patient when I lifted his left lung while aiding a surgeon.I can still, even at this very minute, feel how his heart was beating to sustain its function. That was the most exciting and the best(est) experience which I had throughout my surgery rotations.

On the other hand, now all the girls are also advised to go for rubella vaccination for prophylaxis.

Since I'm done with rewriting my Internal Lecture, and now I'm free to date with my DELL for a movie before I get myself under the blanket. This is the best weekend too, why?'Cos it's a 3 days weekend..HAPPY VACCINATION DAY people!:)

Back into action.

* I managed to take a snap on this rare plant on my way back from psychiatry hosp..what plant/flower is it anyway?*

Good bye Psychiatry and it's time for Orthopedics and Traumatology rotation tomorrow!Dr.Irina Ivanovna Kitaeva delievered our last lecture today after glaring at us upon our answers on "we had copied this materials on our 1st lecture!"..So she continued on theory on emotions and after 1 hour, she left us with the best Psychotherapist in Nizhny Novgorod(well that's what she said)..he's a man in his 50s,good stature and with spectacles.Okay,so you see came about 10 patients(post neurotic patients whoa re under his therapy). They sat on the front rows, and the therapy started with him turning on his radio, asking all of us to close our eyes, relax and follow his commands..okay here you go,I,as usual cannot sit still and yes,I wasn't under his 'hypnotism' but at that very moment lava sms-ed,so I was busy replying that I was wide awake throughout his 30 minutes +++ psychotherapy..well not only me who wasn;t interested but few of us were fidgetting too,* (ha-ha) fil,i saw you completing your summary on russian text!*Moreover all his commands were in Russian,so that was a big turn off for me.As for the enthusiastics, few of them actually ALMOST drifted to sleep!!!

Allright,I know I slacked right after my tb and internal exams..slacked in sense of I wasn;t being very disciplined though I'm still following rules imposed on me by myself. So this is it..freedom and kidding around stops today,gosh..it's already 10th November'09 and I still have lots to accomplish..what happened to all your 'doctor's recommendations?'..okay,since everything is back to its place now,I shouldn;t delay things anymore.I can;t stick to my words without a proper mechanism to control me.Finally,I managed to divide my time and like I had determined before returning to Russian Fed,"it's less than 2 years to go,and I'm going to make sure that I turn out to be a competent one..I will!'I'm going to work for it and any other distractions will not be entertained!*

Transition Period.

1st Transition Period.

-related to my psychiatry rotation. My last tutorial class was today with Dr.Irina Ivanovna Kitaeva. We love her..yes though she makes us sit in that freaking cold classroom for hours during our discussions.I enjoyed my classes with her especially the boys..on a special note,Abdalla, yes the one who loves to ask questions in return before the psychiatrist could ask him!He just asks lots and lots of questions,at times it's acceptable but most of the times you'll go.."abdalla...." because of his too inquisitive act will prolong the time and soon you'll be seeing us gritting our teeths and clutching our fists to box him! However, his questions do make sense, particularly the ones we tend to forget to enquire further explanation. On 1st day of tutorial,we learned on the introductory topic on psychiatry and guess what he asked towards the end?"Dr,how would you diagnose a patient from post traumatic accidents?" Dr: 1st looked at him.Secondly answered"We call that situation chronic/acute post traumatic stress disorder..and we will be discussing about it the next few days!"

-basically,she's an associate professor with a wide knowledge on her field.I'm drawn to the way she will give an example on our daily discussion on certain topics. And each and every example related to her life and her profession, mainly her patients. This semester,we are more exposed on how to interview the psychotic patients, watch interviews and finally like today,we watched "Beautiful Mind" in class as the movie is enclosed with concepts of visual hallucinations, delusions, social phobia, etc.I regeretted for not watching the movie completely previously on my own for I remember I got bored after 30minutes I played the movie.Interesting movie indeed, maybe I shouldn't make an early remark while watching any movies in first 30 minutes!

-Before the start of my psychiatry rotation,I said I wanted to learn and get into certain persons' heads just to know how and what they are thinking right?I think i did get near to my objective,at least now I can differentiate schizoid, schizotypal, paranoid, histrionic(just to name a few) personality disorders. It's interesting to know how you can manipulate someone(patient) to get their life story out of them in diagnosing their illness.Alas,I don;t know how I will be reacting next semester when I were to approach the patients on my own...now bear in mind please, psychotics are entirely different from the ones one see in medical departments!*perhaps,it won't be that bad,Nita..still errr....* But what's vital here in curing these patients ,I think is to provide a good support, love and psychotherapy aid with a great compliance from patients.

2nd Transition Period.

-this is totally revolves around my besties.2 days ago,Subha posted her shout out in fb" Love is in the air"..and yes she received many comments including from me.Though she denied saying it's meant for everyone out there but I as usual, rekindled her and refuse to believe(ha-ha).Later on,in my nap,I dreamt of Lava getting hitched/engaged..now this is the best one, don't you agree with me?I got so excited that before I went to bed today at 3 am,I just had the gut to post it on my fb as my shout out and Miss Lavania became the overnight fame with the total of 71 comments on my post from my besties!!! Everyone seems just too excited, just like me,waiting for her big day to arrive!yeay...

-From the little school girls,we had grown up to young ladies now,we can see the many changes which has come our way.Well,I can see what I had gone through over the years.I know what I want in my life though. When my besties(3 of them now are officially already TAKEN) are planning and enjoying their new moments in life, few of us including me here, still remaining so composed..and why is that so?" Got calon ready ke?"--the usual question being asked and the one and only safe and most appropriate answer of mine for now will be -- Just a smile, which will leave the many questions in peoples minds.Well, hold on to current episode, the right time will come ;)And yeah,I remember once me,shalu and aud said we will go hunting from north to south in order to find Mr.Right for ourselves. Now,isn't that funny?Whichever way life may take us,the only wish/hope I have for my girls "whichever path life may lead us in future, the chain of friendship should never be broken, even in the presence of a new person in our lifes..our friendship is meant to be forever and we are born to be friends till the end" .
p/s:by the way according to my psychiatric mentor,"LOVE IS A PROCESS"-- quote of the day, maybe?

Love you girls and subha, anne and lava---> good luck and all the best girls, make sure to be the best and definitely a sincere and a trustworthy one to your another half.

Miss you all loads.

*a big bear hug to all of you dearies!*

4th Rotation: Psychiatry..(not a good start!)

I had a real fudged up day today! It could never get any worse that this.I always anticipate my new rotations with high expectations only to realise few mentors are simply not cut off for that!I just hate that.I was all buckled up for today's new rotation but I only had to endure all the bad lucks...

First of all,I love my groupmates but when it comes to punctuality, they never live up to my expectations.I'm this person who value time punctuality anytime, anywhere.Guess the years of being prefect back in school trained me into a real good one when it concerns with time.I still can tolerate if you are late 10minutes but not any longer than that.So,2 of my Kenyan groupmates, came out late from their rooms. We headed towards the bus stop, flagged down bus no.43.We didn;t wait for the other 2 JPAs and a Mara because there was no sms-es saying "guys, please wait for us tomorrow" when I remembered yesterday we did tell them that we will be going to psychiatry hospital together. As we wanted to jump another bus,37 which uses another less jammed road,we got down at the next stop. The bus was just behind us but as we got nearer, the door's shut automatically..okay this another scene which you can see frequently while boarding the bus here-I;m kind of used to this because undeniably,Russians let say quarter of them have this dominant gene called 'stupidity' encoded in their sex chromosomes..so now you know,it's passed from generation to generation and there;s nothing much can be done on that fact.

Oh did i tellyou that?To make things worse, today it snowed and pouring at the same time!I would have preferred a cold weather in Malaysia but here it just gets too extreme you know though at times I simply love it.But,the pre-winter weather came too soon this year, hey it's not even November and now you will be seeing this fluctuating temperature everyday which can make loads more sick and unwell students...*sigh*..so when the bus door was closed on our faces,we had to wait for another bus which uses the same route,No.85.Luckily,that bus came earlier. We didn't want to board No.1 as it goes through the city which will take more longer and the traffic which wasn;t budging an inch,we decided to get to Covetskaya Sq.

Less traffic as we had expected, jumped into No.41 but just to note, the door was shut before we could get in but the good samaritan(the bus driver),let us in..Oh,I was so thankful!!!Then in 20 minutes time(at 9am) we reached the Sovetskaya Sq.Once my feet touched down the ground,I was hurrying followed by Jeff,Abdalla,Zakiah and Sara trying to keep up with my pace.I have been to that hospital for my psychology lecture once in 2nd year.I knew the way but wasn;t sure which building.I knew Groups D and F had reached earlier..that's not good..As we entered the main gate,we saw Wani,Shahrul and Fil were just ahead of us few meters."Right then,we're not late",I told myself. After climbing up the stairs to 5th floor,to my relief,I saw everyone infact just reached and were cladding their white coats!!! Choosing the other route unlike the others indeed made us reach the hospital on time like any of them despite being the latest to board the bus.

So you know it's psychiatry hospital.I should have taken a picture of the so called classroom where our lecture was held.Wait, was it even called a lecture when all we did was copying down the 'copy paste' lecture material from the net on our books?I was like sitting about 2 meters away from the lap top containing the lecture materials since the front seats were occupied. The lecturer/a psychiatric obviously not the lecturer material type,no I don;t want to put a bad comment here on him.Thank God,I won't be getting him for my tutorials though I know I will be getting the most strictest one tomorrow,a lady.My group is always blessed with all the big shots and the best ones ever since our 1st year..so I'm okay with it.Okay, let me tell you why I really got pissed with him..he wasn't delievering lecture in the very first place just because he was busy!He came in,read out at super speed, being so inconsiderate of us who were trying to copy down the lectures.I won;t be complaining if it I could see the lap top screen but with the many heads peeking over infront of me,I just gave up at one point and I was at my worst temper!

Today's lecture was the worst one ever, even chemistry lecture by the Russian Professor in 1st year was loads more better and ineresting.Where's the sense,Dr I mean please don't tell us you don;t have a projector to screen you lecture materials for us!As I was trying to cope with it,2 assistants(maybe?),hopefully they are not psychiatrics came into the room to check on us when the lecturer was away..and they made things even worse my just chit-chatting while us trying to hear abdalla who was reading the lecture material. At one point, his voice just drowned with them raising voice,IGNORING US COMPLETELY! I really felt like telling them on their face "SHUT UP and GET OUT".Though we were making faces and indirectly saying 'shut up,stupid,go out' seems it didn't reach to their cochlears..arrggggggggghhhhhh!!!!By that time,I was already at the peak of my temper.

The lecturer entered again, tried to speed up his lectures and I snapped my pen on my notebook and just folded my arms.I came back home and rewrote the 15 minutes lecture which I left unattended.Once he left,we resumed back our copying lecrure when abdalla and sevvanthee took charge by reading them out loud.We did go an extra mile by saving all the lecture materials in Filzah's pendrive!

Thirdly,NEVER consume any water if you will be having classes in the psychiatry hospital.Every single doors to the department are closed tightly, you can never get into them easily even when you are in a desperate situation to go to the ladies.I truly understand, the mentally unwell patients are dangerous at large but atleast they should have been ladies/gents nearby right? How can you actually control your involuntary act? What if your urinary bladder just explodes?Shoot!!!!Damned!God save us for the next 7 days when we are in that hospital!

See,how doomed my day was today? So what's next for tomorrow?..duh..hopefully,we won't be chased around the hospital by some psychics!

Internal Medicine: DONE.

Here I am,after a 3 hours of nap(yeah,i know it's pretty long but my internal rotation is over today!)..all rejunevated, sipping a glass of apple juice and munching my diwali cookies for dinner.;)

Internal medicine this semester was split into two periods-one right after my infectious paediatrics, for 8 days and will be continuing again in december for another 5 days, just before my last cycle for the sem,haematology..and we don;t know who will be taking us because Dr.Ksenia said it wouldn;t be her.Actually i prefer for a change different Dr. each semester.I have been under Dr.Ksenia,Dr.Yulia,Dr.'Nicole Kidman,Dr.Pavlovna..so actually I'm eyeing and really really hoping for Dr.Tatyana for my next semester.Really wish can be under her for the next 5 days for Endocrinology topic.Besides her,I won;t mind if I get Dr.Andrevna or maybe Dr.Panova?Let's cross my fingers for the best..after all it all depends on you no matter how excellent the mentors are.

But,this internal medicine was kind of relaxing for me!Okay I know you must be thinking I'm crazy here for even saying that..well it was seriously because the only topic which I learned in detail would be pulmonary embolism..the rest of the topics were repetition, but with Dr.Ksenia she won;t make life easier but at the same time you will feel a pinch of stress during Q&A session.Just like today, she managed to kinda'screw all of us' on our last day of tutorial class with her.

She returned our case report with comment "I'm not satisfied"..duh! As for me I think,we could've done better if we had written the whole complete case report rather than just writing about arterial hypertension when the patient ACTUALLY got admitted to the hospital due to hyperglycemic attack. So the chief complaints would be?Like I had said, sometimes long and complex process can make the job loads more simpler.Just like my previous patient,he had Diabetes Mellitus Type II(which was the reason for his admittance to the hospital),Arterial Hypertension(type?) and Chronic gastritis. So I had to only write on Arterial Hypertension! Moreover, according to the patient both his arterial hypertension and diabetes mellitus were discovered at the same time,6 years ago. Now,tell me which one lead to another? But one thing,it wasn;t secondary arterial hypertension according to her.*my turn off mood*.

Apart for the case reports issue, normally she would ask us back on the previous materials, and will give points based on how we had answered.Today,I prepared for Cor Pulmonale(right sided heart failure) and Myocarditis(inflammation of the heart's myocardium). She turned the table and here we go,Syahrul,Filzah,Jeff and I were asked on Chronic Bronchitis which was our today's class' topic..ermm,managed somehow but wasn;t excellent. This woman,unpredictable-la...

In spite of all that,we even made a very short case report in an hour time on patient's with chronic bronchitis. My patient,58 years old,a policeman by profession with moderate severity(concluded that after my clarking). He is a chronic smoker for the past 40 year with about 15 cigarettes/day.I'm sure he has complication of Cor Pulmonale but he said the doctor in charge didn;t mention anything about it and I,Abdalla and Zakiah were wondering whether we should omit the complications..moreover we couldn;t be 100% sure because there wasn;t Chest X-ray in his case report. But I did check his ECG and I'm pretty sure I saw few abnormalities, deep S-wave in V1-V6,sinus bradycardia, but there was no right axis deviation indicating right heart involvement. In addition to that, pulmonary function test wan't performed on him on his current admission.I suppose it was done way too earlier when he had his first attack 20 years ago.He too has Arterial Hypertension.

After collecting all the informations needed, the last 1 hour of our tutorial class was discussed on our last patients for the sem,all together 3 patients.Atleast, that part of class went smoothly and finally she gave us her smile,we said 'thank you' and left her, greatfully indeed!

So for now,I'm done with internal..and tomorrow at 8.15am,I will be off for my new rotation,Psychiatry.I'm looking forward for the cycle because I desperately need to know more about people, atleast selected ones which are making my life less cheerful now.Hopefully, towards the end of the rotation,I will be able to read people's minds BETTER than ever.

Lecture Blues

*our anatomy lecture hall..p/s: look, how thin Kok Meng was..lol*

Weekend was 'joyful' after we learnt today will be lectures on infectious paediatrics. To double the happiness(not really),tomorrow will also be lectures...which means we will be having materials before hand for practical classes. That brought smile to many till group 531D are having their 'pot luck' dinner at nana's room tonight..it's like having a day off between the cycles:p

5th year..time table certainly less packed than previous years but lots of readings has to be done. Why am I saying that?-in a week,I only have 1 lecture day of Internal Medicine/haematology on tuesdays with an exception of russian classes on mondays. So,only 2 days of 'extra classes' in a week.*wink*

Well that brings back memories of 1st and 2nd years when once we used to have lectures PRACTICALLY every day right after practical classes(duh,it was really taxing!)

physics: attendance is all that matters, half of the students will be dozing off,and the rest will be coping down the lecture without what it actually means!! Okay sorry to say but physics has never been my favourite ever since school days...moreover the prof. only talks to the tables, chairs and thw white board..

biology:with the vice dean, mdm.elena,it's the same snenario at the anatomy lecture hall..a few will be zzzzz and the rest will be so into coping her lectures..

anatomy: was the best lecture ever with mdm.stenlikova..she makes the lecture and the subject more interesting."kill you handphones"- is her well known phrase.

histology:Who wants to mess with Dr.Radaev?Just the sight of him will send chills down your spine. No,he;s not mean but if you are one second late for his lecture..there you go,-outta the room.Controls/tests with him is what you will never forget..the moment he looks out the window, you know "shit,i have to repeat this test" before you could hear his "To my sorrow, you may repeat!"

Chemistry: The 2nd least lecture that was attended by the students. The prof. is very good but the only problem was his command of english, other than that he was okay.

Medical Philosophy: by Dr.Izutkin--he is a perfectionist,I would say.He will try to manipulate in each and every debate that you have with him.

Physiology:Dr.Mukhina is the person responsible and I actually enjoyed all her lectures and lucky for me to get her as my examiner in exam:)

Biochem:Sir Pavel is like a grandfather whom I've never had.We loved him very much.In comparison to the dean,Mdm. Erlikina, for me Sir.Pavel is way better.

Nursing(uhod za balnimi): only went for the 1st lecture after that none of us attended the next 2 or 3 lectures because we already had the lecture material and the surgeon coughs out exactly what's in the lecture material. And because of our collective decisions,Group B was fired on our behalve since the attendance was too poor!!!

3rd and 4th year:

-perhaps only 3 days/ week we had lectures.

pharmacology: Mdm Dugina is the whiz of pharmaco and drugs.Though the number of lectures should be mor ethan 10,i can merely count about 3 or 4 lectures were conducted in alternate weeks due to her unsatisfactory health condition.I still remember her 'snow ball' game in class and her best quote"Discipline is the mother of success" and she is the only one who will address her students--"Dr.Navanita,please come out and explain this".Thanks,ma'am.

patho physiology: for both semesters we had Sir.Constantine(cekap right his name, just like he himself). He's another grandpa whom you will wish to have for,lol..he loves explaining and never get tired of it.Secretly, all of us agreed he must have been a REAL HUNK during his young days.

patho anat: only the very 1st lecture was conducted by the most feared,Mdm.Artifeksova..and most of it by Sir.Kuznetsov@Chip Munk wherelse infectious diseases by "Gaspadi"..opps,sorry I can never get her name into my mind!

Hygiene:the subject which all of us will go.."oh no"..simply because we never find it;s the most important thing in medic life : what;'s the ideal distance between 2 beds?,what's the hygienic requirement for air humidity ina room?"..arggghhh..but just like sevvanthee would say "there can never be anyone as nerdy/as crazy as us..even for hygiene WE made our own answer schemes!!!" ..*a slap on my forehead..haha*

Medical law: this is another lecture which you cannot forget no matter how boring it is because we have state exam for this material in our 6th year..oh,Lord..

and now 5th year:

Internal Medicine is the king of all lectures ever since 3rd year. The group of lecturers for 3rd year were Dr.Pavlovna and Dr.Tatyana.Dr.Andrevna delivered 1 or 2 lectures and I was her victim on her very first lecture of "ECG" interpretation..well that time I knew nothing about ECG and I know I showed the wrong wave and better still,I couldn;t recognise what was the wave she was pointing at!After that incident,I will chose not to sit right in front of her but things certainly have changed now!

4th year still the very faithful Dr.Tatyana,Dr.Panova(cardiologist) and Dr.Andrevna were the panel of lecturers.

5th year: The first 2 weeks of 10th sem was Dr.Panavo.I enjoy writing her lecture, looking at her slides and listening to her. Then came Haematology lecture with Dr..ermm,sorry,can;t remember though I;ve been attending her lectures for the past 3 weeks.Haematologist with a more oriented lecture, more like M'sian style..we are supposed to print out lecture materials from her web and just 'add on' infos during lectures.

And know what?I find myself pretty discouraged,I don;t know why..maybe it is true that you only can concentrate for the first 40 minutes in each class!! ahh..I will try to keep my eyes wide open and get into her lectures..can;t escape because I sit at my permanent lecture seat..2nd row which is a very strategic place for any lectures to lay their eyes on me.

No offence, but I prefer Dr.Tatyana,Dr.Panavo,Dr.Pavlovna or even Dr.Andrevna;s lectures' methods..where you have to write each and every single word of theirs without non-stop.I think that keeps my attention more focused and I don't drift away by the call of 'vibes'.

Anyways, without lectures I think I will be just rotting in my room either by sleeping or watching movies,maybe?Okay..so what''s for haemato lecture tomorrow?:)
